 How do I replace my NYS photo driver license, learner permit or non-driver photo ID card if it is lost, stolen or damaged?
 You can apply to the Department of Motor Vehicles for a duplicate photo document.
You can apply for a duplicate driver license or learner permit on-line, by phone, by mail or at a local DMV office. You can apply for a duplicate non-driver photo ID card by phone, by mail or at a local DMV office.
The DMV web site has the instructions to replace a NYS photo document issued by the DMV.
