Who investigates unusual or reportable incidents that occur in New York State correctional facilities?

 The New York State Commission of Correction investigates all reportable unusual incidents that occur in any correctional facility in New York State. Additionally, the New York State Commission of Correction has a Forensic Medical Unit which investigates incidents such as inmate deaths and medical or mental health treatment in New York State correctional facilities.
